Supports Coordination Supports Coordination is a service that is provided to all individuals registered within the mental retardation system. A Supports Coordinator is assigned to assist individuals and their families in locating, coordinating and monitoring community and Mental Retardation system services and supports which will support their everyday needs as well as insure their health and safety. Residential Services Residential services are provided in homes and apartments located within the community. Supports are offered based on individual need and range from 24 hour, 7 day a week support to minimal assistance. Life Sharing through Family Living Life Sharing though Family Living provides the opportunity for an individual to reside and participate as a member of a family. The family provides assistance with their daily living needs and the individual shares in various life experiences such as birthdays, weddings, graduations, etc. Supported Living Supported Living provides residential supports to an individual who resides independently in their own home or apartment. Residential support staff provide assistance as needed to help the individual develop the skills necessary for independent living such as budgeting, house cleaning and the scheduling of medical appointments. Community Habilitation Community habilitation is provided in an Adult Training Facility and offers individual assistance and training in the areas of daily living skills, pre-vocational skills and community involvement while also providing recreational and social opportunities. Pre-Vocational Services Pre-Vocational Services are designed to assist persons in establishing and achieving their highest level of social and vocational performance with the end result being to secure meaningful employment. Transitional Work Services Transitional Work Services are offered in a work setting with supervision provided to assist with and oversee all aspects of work performance. Home and Community Habilitation Home and Community Habilitation services are supports provided to individuals and/or their families to assist in maintaining the individual or family member in the home while increasing their independence in the community. Services can include sitter/aide services, respite care and home adaptations. Family Driven-Family Support Services (FSS) FSS offers individuals living independently or with their families the opportunity to purchase items to enhance their ability to remain in their homes and increase community integration skills. Services that can be purchased include respite care, sitter/companion service, homemaker service, in-home therapy, family education/training and recreation/leisure activities. Specialized Therapies Specialized therapies are delivered by a health/behavior professional and include services such as speech therapy, physical therapy, occupational therapy, nursing services and behavior management. Agency with Choice Intermediary Service Organization (ISO) Individuals or their representative contract with a provider agency to act as the employer of record for a support worker who is providing services. The individual or their representative are active participants in the hiring and firing, scheduling, training and also assist in determining the tasks to be performed by the support worker. |
