 Case Management Administrative

Blended Case Managers
help children who have emotional or behavioral problems and their families find local services such as counseling; medical services; recreation programs; tutoring; housing. Supports Coordinators (SC) assist children with
developmental disabilities and their families in locating, coordinating and monitoring services received through the
developmental disabilities system or in the community. Persons with mental health diagnoses may have an SC if they also have a diagnosis of
developmental disabilities.
Early Intervention Early Intervention Services are available to meet the individual and family needs of infants and toddlers
from birth to their third birthday who have been
evaluated as having a developmental delay. Crisis Intervention Counselors are available in the office or over the phone to speak to adults or children who are feeling overwhelmed
or frightened. The 24-hour crisis phone number is (724) 437-1003. Behavioral Health Rehabilitation Services (BHRS) These services help children and adolescents with serious emotional or behavioral problems to be more successful at home, in school, or within other community settings.
These services must be prescribed by
a psychiatrist or a licensed psychologist. Outpatient Psychiatric Services Many children and adolescents experience challenges at home, at school or in the community. Such issues may be successfully treated in an outpatient setting. Outpatient services can include individual and group counseling or medical evaluations. All of these services are
provided by individuals/agencies in or near
Fayette County. Inpatient Hospitalization There are several psychiatric inpatient hospitals within 50 miles of Fayette County that provide 24-hour psychiatric care for children and adolescents.

Family Driven Services (FDS) and Family Support Services (FSS) Family Support Services provides reimbursement for certain expenses necessary to your child's treatment, such as babysitting, respite, recreational activities and more. Family Driven
Services provides an allocation to assist families of children with
developmental disabilities in meeting the needs of the child. Services purchased can include sitter/aide; respite; homemaker; adaptive equipment; and recreational/ social activities. Advocacy/Support Programs The Consumer/Family Satisfaction Team (CFST) seeks input from consumers of mental health services
or their families to see if they are pleased with their services. The CFST uses the data they collect to help assure that the residents of Fayette County are receiving high quality mental health services. The CFST can be directly contacted at the Mental Health Association

helping

Partial Hospitalization Children or adolescents attend a
partial day or
day-long program (under the direction of a psychiatrist) that includes mental health treatment and
an educational program. These
programs are usually
held Monday through Friday and
are intended to help children avoid hospitalizations or out-of-home placements. Transportation is provided by the child’s school district, and the educational credits are transferable to the home school upon discharge from the program. Family-Based Mental Health Services Family therapy services are provided in the child’s home by a team of
two professionals trained in structural family therapy. The team works with everyone in the family to improve relationships and to help the family take advantage of other resources in their community. The team can also help the child and family deal with problems at school and in the community. Several agencies in Fayette County offer this level of care. Home-Based Services
Children with
developmental disabilities and their families can receive in-home assistance
to address the needs of their special needs family member. Services can
include sitter/aide services, respite and home adaptations.
 Mental Health Residential Services In some cases, children or adolescents may need time away from their home in order to benefit from treatment. Available options include residential treatment facilities, community residential rehabilitation foster homes, and youth shelter programs. Parents remain actively involved in the planning and delivery of treatment services for their child
while the child is away as well as being involved in planning their child's return home. |
